Who is eligible?
We adhere to Wisconsin's Department of Health Services guidelines to determine who is eligible to receive vaccines. See their website for additional information.
** if someone is not on the eligible list (listed to the left) we can put them on the waiting list, and they will receive a phone call/email to receive the vaccine if there are no-shows for any appointments.
What vaccine do we administer?
We are currently administering the Pfizer-BioNTech vaccine, which may change moving forward.

Please bring the following to your appointment:
Health Insurance card
EpiPen (if needed)
Why GET TestED Before Vaccination?
The CDC advises against receiving the vaccine if you have a current COVID-19 infection. We do not want to put anyone’s health at risk by administering the vaccine to anyone with a current COVID-19 infection.
We will schedule your second dose after the initial appointment in accordance with CDC guidelines. For more information, see the CDC website for up-to-date guidelines.
Pfizer-BioNTech vaccine: appointments will be 3 weeks (21 days) apart. If you need to re-schedule, there is a grace period of 4 days earlier and up to 6 weeks (42 days) after the first dose
