COVID-19 Vaccination

Summit Clinical Laboratories is partnering with Muskego-Norway School District to offer COVID-19 vaccines for students, faculty & staff, and their families

Who is eligible?

This vaccine clinic is limited to the following:

  • MNS students 16 or 17 years of age with parent consent


  • MNS students that are 18 years of age or older​

  • MNS faculty/staff members

  • Family member of MNS student or faculty/staff member and is 16 or 17 years of age


  • Family member of MNS student or faculty/staff members and is 18 years of age or older

Where and when is the vaccine clinic?

This vaccine clinic will be held at Muskego High School

  • FIRST DOSE: May 13, 2021, 8:00 AM–2:30 PM

  • SECOND DOSE: June 3, 2021, 8:00 AM–2:30 PM

Appointments will take about 30 minutes and registered students will be excused from class.

When is your second dose?

Your second vaccine dose will be scheduled for the same time slot as your first dose and in accordance with CDC guidelines. This registration requires that you register for both the first and second doses on the date, time, and location indicated above. The informed consent applies to both the first dose and the second dose.

Pfizer-BioNTech vaccine: appointments will be 3 weeks (21 days) apart. If you need to re-schedule, there is a grace period of 4 days earlier and up to 6 weeks (42 days) after the first dose.

WHAT Do YOU need for your appointment?

Please bring the following to your appointment: 

  • ID

  • Health Insurance card

  • EpiPen (if needed)

  • Parental Consent form (if needed)

Why GET TestED Before Vaccination?

The CDC advises against receiving the vaccine if you have a current COVID-19 infection. We do not want to put anyone’s health at risk by administering the vaccine to anyone with a current COVID-19 infection.
